Introduced as a diagnosis in 2013, DMDD primarily affects children aged 6 to 18. It involves severe, recurrent tantrums combined with a persistently irritable or angry mood. Children with DMDD often face difficulties in school, friendships, and family life due to poor emotional control, which can significantly affect their growth and development.

Symptoms of DMDD

Symptoms typically include:

  • Persistent irritability or bad temper for most of the day, nearly every day.
  • Severe tantrums three or more times a week, often loud, violent, and out of proportion to the child’s age or situation.
  • Symptoms present across different settings, such as home and school.

For a diagnosis, these symptoms must occur consistently for at least a year, with minimal breaks.

Causes

The exact causes of DMDD are not fully understood. Research suggests that genetic predisposition, environmental stressors, and family difficulties may contribute. Children exposed to chaotic or high-stress home environments may be more vulnerable. Neurobiological factors are also thought to play a role, particularly in areas of the brain responsible for regulating mood and behaviour.

How DMDD Differs From Other Disorders

DMDD shares symptoms with ADHD, Oppositional Defiant Disorder (ODD), and Bipolar Disorder, but differs in frequency and severity. For example:

  • ADHD involves difficulties with attention and focus, sometimes accompanied by hyperactivity.
  • ODD features deliberate defiance, antagonism toward authority, and spiteful behaviour.
  • Bipolar Disorder includes cycles of depressive lows and manic highs, unlike the chronic irritability of DMDD.

Comorbidity is common, particularly with ODD, meaning children may show overlapping symptoms of both conditions. Accurate diagnosis by a qualified clinician is essential to ensure appropriate treatment.

Treatment Options

Cognitive Behavioural Therapy (CBT) is commonly recommended. It helps children recognise anger-inducing thoughts and develop healthier responses. By teaching coping mechanisms, CBT equips children to manage frustrations without escalating into explosive tantrums.

✽ Medication

In severe cases, psychiatrists may prescribe medication. Antidepressants can reduce irritability, while antipsychotics may be considered for extreme outbursts. However, due to potential side effects, therapy is often prioritised before medication is introduced.

How Parents Can Help

Parental understanding is crucial. By identifying triggers, parents can minimise exposure to situations that provoke irritability. Responding with calmness and consistency also provides children with stability.

Teaching relaxation techniques—such as deep breathing, mindfulness, and anger management strategies—can help children regulate emotions during tantrums. Reinforcing positive behaviour with praise and rewards also encourages emotional self-control.
